Upper Canada Family Health Team: a group of health care professionals working collaboratively to provide comprehensive and accessible health care. The Upper Canada Family Health Team has newly opened an Unattached Patient Clinic in Mallorytown for residents of Leeds and Grenville who don’t have a primary health care provider. You are encouraged to continue searching for a local primary health care provider as this opportunity is a temporary solution.
